This policy describes how Buena Salud Acupuncture Clinic handles information collected through buenasalud.day and the appointment and patient forms linked from this website. For information about the use and disclosure of your medical records, please contact the clinic for its Notice of Privacy Practices.
Information you provide
When you contact us, we receive the name, email address or phone number, and message you provide. The contact page provides a general inquiry form, email links, and a telephone number. Our linked booking service collects the information you enter to arrange an appointment. Patient intake and consent forms collect identifying and contact information, health history, and consent responses needed for your care. Please keep medical records and sensitive health details out of the general inquiry form and ordinary email.
How information is used
We use the information you provide to respond to questions, arrange visits, prepare for care, maintain patient records, and administer the clinic. We do not use contact or intake information for advertising, sell it, or disclose it to third parties for their advertising or marketing.
Service providers and disclosures
The website uses hosting, email, security, and form services to operate. Google Forms and Google Calendar are used for linked patient forms and appointment booking. Information submitted through those services is processed by the service and received by the clinic. Necessary service providers may process information to deliver these functions. Medical information may also be disclosed with your authorization or as permitted or required by applicable law, including for treatment, payment, and health care operations. The no-marketing statement above does not prohibit these necessary or legally required disclosures.
Cookies, logs, and external links
Website hosting and security systems may process technical information such as IP address, browser details, requested pages, and access times. WordPress uses cookies for administrative login and related site functions. Your browser can control cookies. This website does not provide a separate response to browser Do Not Track signals. External services have their own privacy and cookie practices; they may collect information when you follow a link and may associate activity with your account if you are signed in. Record retention
Our clinic policy is to retain medical records for at least 10 years after the last date of service. For patients treated as minors, we retain records until the later of 10 years after the last date of service or the patient’s 25th birthday. These are clinic policy minimums, not a statement that one rule applies to every record.
Longer legal, payer, contract, audit, or litigation requirements take priority. For records subject to California Welfare and Institutions Code section 14124.1, the 10-year period runs from the latest of the applicable plan-provider contract end date, audit completion date, or service date. Records subject to an unresolved claim, audit, investigation, or preservation requirement are not destroyed while that requirement applies. Reaching a minimum period does not trigger automatic deletion.
General inquiries that are not part of a medical record are kept as needed to respond, administer the clinic, and meet applicable obligations. This notice does not set an automatic deletion schedule for those messages or technical logs.
Questions and requests
Contact us to request access to or correction of your information, or to ask about retention or deletion. We may need to verify your identity. Medical-record and legal retention duties can limit deletion. Do not send identity documents or medical records in an ordinary email; call first to arrange an appropriate method.
